Chondroitin sulfate and glucosamine combination are examples of a new approach in the treatment of joint problems such as osteo-arthritis or trauma known as chondroprotection.  Contrary to ordinary treatment, chondroprotection is aimed at increasing the defense mechanism and enhancing the reconstruction and self-healing of joint cartilage.  

Chondroitin sulfate is extracted from bovine and fish cartilage.  It is one of the big structural molecules of the body called glycosaminoglycans.  These molecules are found throughout the body: conjunctive tissues, muscles, tendons, etc., in short, anywhere there is  a need for support and structure.  Chondroitin sulfate is a polymer of N-acetyl-galactosamine and glucuronic acid.  It is also called galactosaminoglucuronoglycan sulfate. 

Chondroitin reduces inflammation.  This effect is explained by many action mechanisms.  First, it causes a decline in pro-inflammation prostaglandins.  Then, it inhibits many enzymes secreted by chondrocytes: elastase and yaluronidase.  These enzymes are responsible for the elimination of old cartilage, making room for the new one.  It is a useful and desirable effect in healthy cartilage but becomes out of control in wounded or inflamed joint. Chondroitin also inhibits leukocyte protease, thus reducing white blood cells (leukocytes) activity in the joint.  Leukocyte protease allows white blood cell to destroy wounded tissues to allow for better healing.  Once again, white blood cells have a necessary effect to fight off intruders (bacteria, foreign bodies).  But in the case of chronic inflammation, the immune system starts fighting its own body (autoimmune disease) and damages the tissues it is supposed to protect.

Chondroitin also reduces a process called neo-vascularisation.  The cartilage is not normally irrigated by blood vessels.  In osteo-arthritis, cells from the capillary endothelium migrate to the cartilage matrix, forming new capillary blood vessels under the influence of the angiogenesis factor. Chondroitin inhibits this factor allowing a lowering of the endothelium cells and a return to a healthy cartilage matrix. 

The use of chondroitin sulfate is not recent.  In veterinary medicine, it is often used when treating horses.  These animals can have joint problems due either to weight and lack of exercise , or to joint overuse in racing horses.  These problems are strangely similar to those experienced by humans. Chondroitin provides relief for animals as well as for humans. 

Justifiably, chondroitin sulfate is usually associated with glucosamine sulfate.  Their action mechanisms are related but also complementary.  Their effects do not only add up, they potentialise each other.  They work synergistically to regenerate the wounded cartilage. When these two products are used concomitantly, the dosage ratio (glucosamine : chondroitin) 1:1 seems to be the appropriate balance. 

Chondroitin sulfate is absorbed at 70%.  In the intestinal lining, the liver and the blood, it is metabolized by enzymes into its basic components (N-acetyl galactosamine, glucuronic acid and sulfate).  Even so, 8% of the chondroitin reaches the cartilage whole.
